If you are a student on sponsorship through the Saudi Cultural Bureau, Libyan Government, CBIE, or Kuwait or any other sponsored program you are still required to submit an opt-out application by the deadline.

Eligible Sponsored students must fill and send the opt out form to the SMUSA Health Plan Office at healthplan.smusa@smu.ca. 

To apply to opt-out, you must complete the opt-out application and attach a picture of your health insurance card from your sponsored program.
 

If you do not have a card or require assistance opting-out, contact the SMUSA Health Plan Office BEFORE the deadline.

Fall Term: August 15, 2020 - September 25, 2020
Winter Term: December 15, 2020 - January 25, 2021
Spring/Summer Term: April 15, 2021 - May 25, 2021
 

If you miss the opt out deadline you will need to submit an appeal. For more information on appeals, please contact the SMUSA Health Plan Office directly.
