If you need to use your plan always remember:

 

  • Call before seeking services to ensure you are covered

  • In the event of hospitalization, call within 24 hours or coverage may be reduced

  • Before any surgery, x-rays or diagnostic imaging, please call to confirm coverage.

If you need to see a doctor, please call us to confirm your coverage. We can also help direct you to the nearest medical facility.

 

If you do not call in before seeking services, we cannot guarantee coverage.

 

If you see a doctor, you may be required to pay up front and submit a claim to get reimbursed.

If you are in need of emergent medical attention, please call the local emergency number.

 

If you are admitted to the hospital, please call or have someone call on your behalf within 48 hours.

 

Insurance coverage may be reduced if the insurer is not contacted within a reasonable time frame such as 48 hours.

 

Before seeking any major medical treatment such as surgery, x-rays, diagnostic imaging or lab tests you must contact the insurer directly for pre-approval.

 

If pre-approval is not provided, coverage may be reduced.
