If you have paid up front for medical services (doctor visits, prescription medication,emergency hospital expenses, lab tests, x-rays, etc.), you will need to submit a claim for review for reimbursement (receive money back).

How to Submit an International Claim


If you have paid up front for medical services (doctor visits, emergency hospital expenses, lab tests, x-rays, etc.), you will need to submit a claim for review for reimbursement (receive money back).


NOTE:
  • Accidental / Emergent Dental benefit claims are covered at a dental office provided that the dentist provides supporting documentation that care was received due to an accident or emergency. If this documentation is not received at time of claim there will be delays with processing as we wait for this information to be made available.
  • Travel vaccines are not eligible for coverage unless you require it for a school course. In order to review the claim for eligibility please include supporting documentation that your course required the vaccine.

(New) By Online Claim Form:
Please click here to submit your claims virtually and upload any supporting receipts, medical notes, prescriptions, etc.
NOTE: If you have a Canadian Bank Account, please upload a copy of your void cheque for faster reimbursement.

* Failure to complete and sign this form in its entirety or submit supporting documentation will delay claim processing.

 

TIPS
  • Ensure you complete as much of the form as you can to avoid delays in processing. You must include your name, student ID number and contact information
  • Include the invoice or receipt from your medical appointment with your claim form.
  • Include any medical notes, doctor notes, lab results or prescriptions to support your claim.
  • If mailing your claim, please keep copies of all of your documents.
Medavie Blue Cross™ will issue reimbursements in the form of a paper cheque, to the address on file, if there is no direct deposit information provided. Please note that cheques are considered stale dated after six months unless certified. If your cheque is stale dated within 12 months of the end of the policy period in which your claim was incurred, you can contact Medavie Blue Cross directly to have the cheque reissued. If you are requesting to reissue a stale dated cheque that is beyond 12 months from the end of the policy year in which the claim was incurred, you must contact Student VIP directly. Any requests made outside of this timeframe may not be honoured.

Manual: Fill out a Direct Deposit Form and send it to Medavie Blue Cross via email (acadia@medavie.bluecross.ca) or mail it to the closest Blue Cross office address listed on the form



Instructions on how to register for My Peak

1. Download the My Peak app in the App or Google Play Store
2. Select ‘Create’ Account and use group code ASUINTL
3. Click the link in your email and login to the app
4. Select the wallet card icon on the bottom right
5. Select Health Plan Card and complete the fields
