If you have paid up front for medical services (doctor visits, emergency hospital expenses, lab tests, x-rays, etc.), you will need to submit a claim for review for reimbursement (receive money back).
If you have receipts for prescription drugs, dental, vision, or paramedical (massage, physio, etc.) expenses please submit these to the SMUSA Extended Health & Dental Plan – information on how to submit this type of claim can be found here.
